3 It completely depends on the kid and the phone's capabilities. Overall I'd say 3 but move up or down depending on the kid and type of phone.

3 I interpret the question to be about kids having their own cell phone attached to a phone plan, etc. My kids do have phones, but they are only used for photos and Pokemon GO and they are old phones we had laying around.

1 Daughter (now 12) got her iPod at 2, first tablet at 8. Phone at 10 (no sim). Gradually extending limits, screen time and apps installed. A year ago got a SIM too. Very healthy relationship with the device.
