Is there a phone OS designed for children? I would expect a restricted by default OS where all options (and what apps/domains can be accessed) are managed by a parent device.

Probably not the answer you're looking for but Android is very good for locking down devices very tightly imho. That's what I chose to use for my kids at home.

When I looked into it, most were 10 year old unpatchable Android devices. We ended up just going with a Galaxy Tab A7 Lite and locking it down with Google Family Link, which has been good. Wish it had more space for downloaded media, but 10 inch tablets seemed too big. We got it for her third birthday and she loves it.

I think you can set up iOS for kids so that all downloaded apps and all websites they access need to be approved by a parent device first. My friend has set up something like that for his kids.
