I really don’t get how people don’t have an inner monologue— do they just hear nothing in their heads? This always fascinates me.

It's a lot like if you are "thinking out loud" in front of someone – saying your thoughts as they are emerging to you naturally. An inner monologue is like doing that process, but within your own mind. Sometimes you can actually hear the words but often you can just kind of feel them.

My inner monologue is constant — sometimes reflecting what I’m doing (writing) sometimes reflecting on the world around me. And sometimes having arguments with people in my head.
