Another reason for this is the growth of PAs and NPs in primary care who reflexively refer to specialists for everything Physician PCPs are very comfortable managing most common issues. @farmerboy thoughts?

Financial engineers industrialized health care like ford did through car industry. It’s sad we created technicians with short training who can’t tell 5-10 differentials of most common disease . It’s very sensitive zone to talk about even . We can’t criticize one individual knowledge, that’s not proper . But we can criticize looking how system is developed to produce so called skilled worker . When we don’t think we do test and ref every places . More radiation exposure every where through more test . Working in primary care setting I can see everyday billions dollars waste by doing test . We are in tough time . Training being distorted and financial business person don’t understand depth how physician works . Fundamentals training system need revolution. We have reached to the point where we are about to CT scan of hair and don’t know what we are looking even .

It's interesting to contrast your experience with mine talking to rich patients/marketing to consumers of health tech. Some of what you observe must come in part from their demand. I know many people who feel MORE IS ALWAYS BETTER and this includes testing. They feel entitled to these services and are unwilling to accept that that it's a waste in the slightest

Totally agree. The word “” feel entitled “ is society empowered on them . In USA every 30 min there is one add from attorney on Tv , “ call me and sue physician, I will get you money “ not sure how many country in the world has such sick advertising to attack professional doctors . No doubt we do defensive medicine practice. Medicaid - 21% USA population Medicare 18.7% Private insurance: 54.5% Probably you see more third category insurance coverage patients 😂. I see more first two category. I think always if I can save government money though government don’t care for us .
