Question to ask an engineer that gives the best signal?

There's no silver bullet question for having a good hire, but in general I ask everyone: 
- What is the biggest mistake you've ever made in? = copout answer means they're high ego, honest answer is low ego.
- Teach me the most difficult concept you know in 5 min

They should meet the team and get along with everyone :)

It means you can acknowledge your mistakes, and in this case, a very big fuckup.

If someone thought so highly of themselves, would they admit making a mistake at all?

Im not sure what you mean by skipping the question, but I do think your last point is correct.

It’s a skill to get people to be honest with you and bring the best of them out!

What about any other factor that may have influence on the fact of skipping the question?

I skip a lot of questions for many different factors, and most of the time is not about the ego.

Also, an answer does not have to be honest.

Don't mind me, I just think it is more a skill you have, than the questions you ask.
