Reading about the August 2020 Beirut explosion which seems like multi-level protocol failure at the intersection of a failed state and a failed (bankrupt) shipping company. I didn’t pay attention at the time since it seemed part of the general apocalyptic mess of the pandemic but this is worth a movie. It’s a perfect case study in how complex systems fail. More layers of failure than even nuclear accidents.
