why? how come that hasn't happened with ACH?

I think because FedNow settles instantly and it is non-reversible so it is much more likely for businesses to use it. I think the problem with ACH is that you can do transactions like allow buying Crypto because it is reversible and because of that companies have problems with fraud https://i.imgur.com/F0LHuI3.png

Fraud is still just as big of a problem without reversible transactions.  The question is who accepts liability
