This is a combination of the lag between system design and system implementation

This is also a symptom of systems being designed in waterfall fashion instead of iterative

I’ll buy that combo, I’m not sure iterative solves it for it so much as it wishes it did. I’d argue iterative itself falls short when it comes to managing the future of a system but it does manage adjustment within the existing system better
