Not sure I'd agree all mediating hierarchies are flawed or intrinsically pathological. My reading Kay sees the mediating hierarchy as scaffolding for mutual gain and collective intelligence.

+1. Haven't read the book yet, but putting my workplace safety hat on, I'd guess that mediating hierarchies generate a lot of protocols that manage the tensions between the trade-off preferences of labor and capital. Like safety protocols, which are mutually beneficial esp. in long run.
