We have a frame in the works where this image "404" also occurred - I'm wondering if something changed in the client? 

Also, not sure if Paragraph supports the Open Frame spec as well, but maybe there's a collision of some kind?

we found a number of our older frames were having this bug at end of last week; @kaushal updated them to the latest frames spec and it fixed them all

Actually just checked, it does seem to be a client issue.
