A company that
- isn’t focused on making products that customers _need_ not want
- prioritizes growth too early before finding PMF
- cares more about butts in seats at set times over quality of work
- doesn’t also prioritize wellbeing of employees
- is too risk averse to innovate

I'd add as part of the risk-aversion: to be too afraid to cannibalize a cash cow. The most amazing bet Apple took was in the iPod, iPhone, and iPad – all, but especially the latter, ate into their Mac line. Amazon in the Kindle, despite their original niche being a book store.
