This is an example - there is no consensus that a validator is matching a liquidation order against the book at the best price possible (this is a result of an off-chain orderbook). In some cases a malicious validator can match the order at such a bad price that the insurance fund takes a hit even though there was liquidity to match the liquidation at a good price that doesn't effect the insurance fund. Repeated and it can drain the insurance fund
