Eh, I usually log into my node a couple of times a month to update either the CL or the EL client based on their release schedule, & I take that opportunity to also do a general OS update. Takes me a few mins each time

I get occasional downtime notifications & they resolve themselves or I (rarely) just reboot the node

It depends on your setup. Eg. if you chose dappnode it updates automatically.
