I basically assume that if they're on farcaster they've passed through some quality filter. So far that's proven to be true. The false positive rate is so much lower than the false negative rate that anyone not following everyone is probably doing so for bad reasons (e.g. to maintain a good "ratio")

It's not a global feed. Because there are certain people I don't follow. Ideally, I'd like to have a tunable global feed where if someone says something interesting I can increase the frequency I see them. This would be a distinct action from hearting their post or following them.
