I can't point to one book as life changing, they all just go in the mix... But I think the one that has stuck with me the most, that resonated with me as deeply important, is Ursula Le Guin's translation of the Tao Te Ching. Paul Stamets Mycelium Running gets an honorable mention.

Tao Te Ching, 1st summer of college...(on acid, does that count?). Solidified my life's purpose to help people. I became a nurse 4 or 5 years later

In high school someone gave me zen and the art of motorcycle maintenance and it sort of created a whole new world view for me.
