Idea #1: Hacker Group 1 performs a zk proof that data was accessed and then e2e transported into a TEE where the data was destroyed after the random payment was received. (I know, there are a lot of failure modes here, it's just a concept)

Idea #2, the less ridiculous one: hospitals and hospital tech providers should not LARP as security experts!!!!!!!!!!!!!!!!!! A new layer of security service providers hold patient data and only provision access (or #FHE computation) to hospitals on-request.
