I've heard two theories as to how NPCs become NPCs. One from Carl Jung which was that they never encountered impossible problems. The other from Ayn Rand which was that they are second handers.

I suppose a third is Mimetic theory from Girard, but this is similar to Ayn Rand's second handers.
