Yes, frequently. It’s an important catalogue of decisions/approvals in my work and becomes a record of truth if any litigation were to happen. 

The “how” is the problem. I can’t find a workflow I like. Might start in textedit, word, notion; ends up as a pdf to the client.

Yes, typically:
Pen/Paper -> Word 

Sometimes:
TextEdit or Notion -> Word

Neither work that well and I can’t tell if it’s my fault (I’m not diligent enough to really go through and refine ALL my notes ALL the time) so there’s a lot of waste in my process.
