Hospital exec in a meeting recently was talking about reducing nocturnist and swing pay because 'it rarely gets busy'. What people dont understand is you dont pay for the quiet nights, you pay for those nights when the house is on fire and the physician is there to let you sleep well at night.

Agreed. I'm seeing a big trend to tele-nocturnist. I see how great that could be for staff hospitalists to not have to rotate through nights but I'd want a lot of data showing patient outcomes aren't adversely affected. Nighttime is when the ghouls come and you need someone who really knows how to work a dumpster fire
