The shower I built was off grid so we used a propane heater. If you have access to natural gas, tankless would be a good idea. 

The shower deck was about 10 inches above grade and put 12 inches of peastone underneath, which allowed for drainage to the shower

Always good to have a bigger tank so people don’t run out of hot water in the shower!
