Last night, a patient in the psychiatric unit where I work was convinced he was being wiretapped and pursued by the mafia. He believed there was a tracking device implanted in his arm and wanted to cut it off and set his car on fire to stop them from following him. He also said he could see drones flying overhead.

It is entirely possible though that he is actually wiretapped and in danger from the mafia which would exacerbate his paranoia.
