some I can think of incl. off-ramping, refunds, taxes, convincing customers to pay fees for transacting

The fees part I've gotten used to as I've used crypto. 
I hadn't even thought of refunds 😳

Is there a flow for this already?

That's true. At least with base the txn fee should be quite low compared to getting recked by the CC transaction fees.

Sending a new transaction sounds rough for accounting but I'm not an expert.

tbf refunds are also quite clunky with cards. I’m not sure how it would work for crypto but transactions are immutable so likely merchant would need to send the refund as a new/separate transaction
