In aviation safety, we use James Reason’s “Swiss cheese” model to describe the successive layers of organizational defenses against human errors (which, themselves, form the overwhelming majority of errors). 1/

never heard of the swiss cheese model before but it makes so much sense! layers of defense to catch errors before they cause major issues is genius. it's like aviation has its own safety net. definitely gonna read more about it. thanks for sharing this, really eye-opening stuff!
