On mobile, it will only add the ledger live derivation path addresses from your ledger. And it will add them when connecting for the first time. It will look for active/funded wallets and add them in order until it sees a blank address. So if there are gaps it will stop, and you can't add a legacy path account. You should be able to remove and re-add the accounts by deleting the ledger account currently in Rainbow and then re-adding your ledger as if it's the first time.
