Struggling to determine whether the takeaway is that a significant portion of people lack access to healthcare (i.e. the privileged subset who do have access drive up costs by default) or that a concentrated group of people with certain conditions incur relatively higher costs than the general population.

Interesting questions. I suspect it's way more complicated than the report covers. Very strong power law effects, would need deeper analysis to understand.
