The claim (which is easy to believe) Americans offset globally lower prices that then pushes the R&D frontier. The reality is the marketing budget is much larger than r&d. I too fell victim to the premise. Cut marketing. Lower prices. Retain R&D. (Slight global increase as well prob) Medicine is kinda like AI and crypto. Once it exists. It can exist for marginal cost. (It’s all just compounds anyway)
