Figured it out. It looks like the 4337 EntryPoint calls RETURNDATACOPY with invalid arguments when a postOp reverts.

Yes, must somehow come from a RETURNDATACOPY. RETURNDATACOPY is the only copy operation that errors on out of bound, whereas all other copy ops - CALLDATACOPY, (EXT)CODECOPY, MCOPY - instead pad with trailing 0s.
