I need a minimal, reusable example of an upload modal in React Native with the following requirements:

Bottom Toolbar: The input accessory or toolbar should be positioned at the bottom of the screen when the keyboard is not visible.

Keyboard Awareness: When the keyboard appears, the toolbar should automatically render directly above the keyboard

Dynamic Positioning: The keyboard offset should be dynamically calculated to accommodate different screen sizes
