I agree with @downshift.eth, the 5 Whys is a great method. Most problems are only viewed at the surface level.

Subject matter experts from various departments and fishbone diagramming can usually root out the real problem. Sometimes though, the SMEs are the problem because they employ workarounds instead of addressing the root. Other times, SMEs aren’t given the resources to do so.
