OK, so Boeing is famously falling apart largely because it has outsourced too much of its manufacturing processes, and lost the ability to control quality. Does outsourcing always have that result? Are there examples of organizations that have done this successfully, and what distinguishes them?

There's a big difference with aerospace vs normal manufacturing. But I can't say that I've seen anything in a manufacturing sector that isn't done better in house. As for the Apple example, they have leveraged a great deal into QC and tolerances (which are rather weak compared to say, arcseconds).
