"Thanks a lot" is used sarcastically, so you shouldn't use it when you're truly grateful.

What?? This expression is taught a lot in Japan... I even remember it being on English tests...

That's not necessarily true. Context, tone and facial expression matters.

If I said "Thanks alot 🙄" Then you would know I'm insincere about it and being sarcastic or a 皮肉屋

But if I said "Thanks alot! 🤩" Then it seems like I'm actually thankful.

Pre and post-sentence leaders can also set the tone. 
For instance "Uhhh.... thanks alot?" "Thanks alot I guess.."
Makes it seem kind of uncertain.

So yeah there is a lot of circumstances that can change the meaning.

Same as "I love it"
